the product can be used with Bioda immune detector to quantitatively detect the parathyroid hormone in human whole blood, serum and plasma, for the differential diagnosis of hypercalcemia and hypocalcemia.

[Detailed information]:

Parathyroid hormone is secreted by parathyroid gland and released into blood. The complete parathyroid hormone consists of a polypeptide chain containing 84 amino acids with a molecular weight of about 9,500 daltons. Its N-terminal biological half-life is only a few minutes. Selective parathyroid hormone detection can be directly used to determine the secretion activity of parathyroid gland. Parathyroid hormone, vitamin D and calcitonin mainly mobilize calcium and phosphorus from the skeletal system, improve the absorption of calcium in the intestine and the excretion of phosphorus by the kidney. The level of blood calcium is kept constant by the interaction of parathyroid hormone and calcitonin. In the case of hypercalcemia, parathyroid hormone secretion will be inhibited, and in the case of hypocalcemia, its secretion will be promoted.

The dysfunction of parathyroid gland will lead to the increase or decrease of blood calcium level (hypercalcemia or hypocalcemia), resulting in the change of PTH secretion.

Hyperparathyroidism often leads to increased PTH secretion (hyperparathyroidism). Primary hyperparathyroidism is mainly caused by parathyroid adenoma. In case of hyperfunction, low blood calcium level is caused by other pathological factors (such as vitamin D deficiency).

Now, the detection of PTH and calcium concentration is of great significance in the evaluation of hyperparathyroidism.

The detection of PTH during the extirpation of parathyroid adenoma has been reported in primary hyperparathyroidism, secondary hyperparathyroidism caused by renal failure, and other types of hyperparathyroidism related to kidney transplantation. Since the half-life of PTH is only 3-5 min, PTH can be used to evaluate whether the hyperactive parathyroid tissue can be completely removed by surgical excision of the parathyroid gland.

The guidelines of National Academy of Clinical Biochemistry (NACB) suggest that initial samples should be collected before surgery and excision of suspected hyperactive glands. PTH is detected within 5-10 min after excision. If the PTH level is more than 50% lower than the highest initial value, the surgery is successful. Additional samples may be required as sensitivity increases over time. If PTH fails to decrease to the recommended level, then:
1) The presence of residual hyperactive tissue may need to be further observed. Generally, two patients in one case have 1/5 ectopia parathyroid to be treated by re-operation.
2) PTH reaches peak in adenoma excision. PTH detection will provide a rapid and reliable result judgment after excision of all the hyperparathyroidism tissues.
